- The Zepbound® (tirzepatide) KwikPen is a multi-dose pen that holds 4 weekly doses, about a month of treatment, at one fixed strength. Pens come in strengths from 2.5 mg to 15 mg.
- Unlike the single-dose Zepbound® pen, the Zepbound® KwikPen needs a separate pen needle for each injection, and you must prime it before every weekly dose.
- Throw the KwikPen away after 4 doses, 30 days after first use, or 30 days at room temperature, whichever comes first, even if medicine is left.
- Lilly announced FDA approval of the KwikPen in February 2026. It holds the same medicine and doses as Zepbound®'s single-dose pen and vials.
Zepbound® is a once-weekly injection for weight loss in adults with obesity, or overweight with a weight-related health condition, and for moderate to severe obstructive sleep apnea in adults with obesity. It now comes in four forms, including the KwikPen.
The KwikPen works differently from the single-dose pen many people started with. Read on for how it works, how to use and store it, and which needles it needs.
Read the Zepbound® Prescribing Information and Medication Guide, including the boxed warning, before starting treatment.
What is the Zepbound® KwikPen?
The Zepbound® KwikPen is a disposable, single-patient-use pen that holds 4 fixed weekly doses of tirzepatide. Each pen comes in one strength, and you inject one full dose each week.
| Dose per injection | Total in each KwikPen | Role in treatment, per the FDA label |
|---|---|---|
| 2.5 mg | 10 mg in 2.4 mL | Starting dose for 4 weeks, not a maintenance dose |
| 5 mg | 20 mg in 2.4 mL | Step-up dose and maintenance dose (weight reduction only) |
| 7.5 mg | 30 mg in 2.4 mL | Step-up dose |
| 10 mg | 40 mg in 2.4 mL | Maintenance dose (weight reduction and sleep apnea) |
| 12.5 mg | 50 mg in 2.4 mL | Step-up dose |
| 15 mg | 60 mg in 2.4 mL | Maintenance and maximum dose |
Your prescriber sets your dose. The label starts everyone at 2.5 mg for 4 weeks. After that, the dose can rise in 2.5 mg steps, at least 4 weeks apart.
The KwikPen is covered by the same FDA label as the other forms of Zepbound®. In the 72-week SURMOUNT-1 trial of 2,539 adults, average weight loss was 15.0% on 5 mg, 19.5% on 10 mg, and 20.9% on 15 mg, compared with 3.1% on placebo.
How is the KwikPen different from the single-dose pen and vials?
The KwikPen holds 4 doses and uses a separate pen needle, while the single-dose pen holds one dose with a built-in needle. Vials need a syringe instead of a pen.
| Feature | KwikPen | Single-dose pen | Vials |
|---|---|---|---|
| Doses per device | 4 | 1 | 1 (single-dose) or 4 (multi-dose) |
| Liquid per dose | 0.6 mL | 0.5 mL | 0.5 mL (single-dose) or 0.6 mL (multi-dose) |
| Needle | Separate pen needle, not included | Built in | New syringe and needle each time |
| How you inject | Attach needle, prime, dial, push and hold | Place on skin, turn the lock ring, press and hold up to 10 seconds | Measure the dose with a syringe |
| Time allowed out of the fridge | Up to 30 days | Up to 21 days | Up to 21 days (single-dose) or 30 days (multi-dose) |
All four forms contain the same medicine. The label says you need training on your specific form, and new training if your prescription changes forms.
How do you use the Zepbound® KwikPen?
To use the Zepbound® KwikPen, you attach a new pen needle, prime the pen, dial the full dose, and inject it under the skin once a week. Follow the official Instructions for Use and the training your provider gives you. The main steps are:
- Check the pen: Make sure the name and strength match your prescription. Don't use it if it's expired, damaged, frozen, cloudy, or discolored.
- Attach a new needle: Wipe the red seal with alcohol, then twist on a new KwikPen-compatible pen needle.
- Prime before every dose: Turn the dose knob 2 clicks, hold the needle up, and gently tap the pen so air bubbles rise to the top. Push the knob in, hold it for a count of 5, and check for a drop at the needle tip.
- Dial your dose: Turn the knob all the way until it stops and the dose icon shows in the window. Don't count clicks.
- Inject: Insert the needle into the skin of your abdomen (belly area), at least 2 inches from the belly button, or into the skin of your thigh. Push the knob in until it stops, hold it for a count of 5, and check that the dose icon shows.
- Remove and store: Replace the outer needle shield, unscrew the needle and put it in a sharps container, then replace the pen cap. Don't store the pen with a needle attached.
If you prefer to inject into the back of your upper arm, have someone else give the injection. Rotate your injection site each week; see our GLP-1 injection site guide.
If no drop appears after priming, repeat it up to 2 more times. Then change the needle and try once more. If it still fails, call Lilly at 1-800-545-5979.
People who are blind or have vision problems shouldn't use the KwikPen without help from someone trained to use it.
Which needles does the Zepbound® KwikPen use?
The Zepbound® KwikPen uses separate KwikPen-compatible pen needles, which aren't included with the pen. Use a new needle for every injection to help prevent infection and blocked needles.
Ask your pharmacist which pen needles fit the KwikPen. Never share your pen with anyone, even with a new needle, because that can spread serious infections. Put used needles in an FDA-cleared sharps container, and follow the FDA's sharps disposal guidance.
How should you store the Zepbound® KwikPen?
Store unopened KwikPens in the refrigerator at 36°F to 46°F, where they last until the expiration date. Don't freeze the pen, and keep it away from heat and light.
After first use, you can keep the KwikPen in the fridge or at room temperature up to 86°F. Throw it away after 4 doses, 30 days after first use, or 30 days at room temperature, whichever comes first, even if medicine is left.
The pen stops you from dialing a full fifth dose. Never inject leftover medicine or move it into a syringe. See our GLP-1 storage and travel guide for trips.
How do you get a Zepbound® KwikPen?
You need a prescription written for the Zepbound® KwikPen specifically. Lilly first offered the KwikPen through its self-pay pharmacy in February 2026, at the same self-pay price as single-dose vials.
On March 16, 2026, Lilly expanded KwikPen self-pay pricing to major pharmacies through a KwikPen savings card. The card doesn't apply to single-dose pens or vials. Insurance coverage varies, so check your plan; see our Zepbound® cost and coverage guide.

Who shouldn't use Zepbound®, and what are the main risks?
Don't use Zepbound® if you or a family member has had medullary thyroid cancer, or if you have MEN 2 (multiple endocrine neoplasia type 2). Also avoid it if you've had a serious allergic reaction to tirzepatide or to any of the ingredients in the product.
Zepbound® has a boxed warning, the FDA's strongest, because tirzepatide caused thyroid tumors in rats. It's not known whether it does this in people. Tell your provider about a neck lump, hoarseness, trouble swallowing, or shortness of breath.
Don't use Zepbound® with other tirzepatide products or any GLP-1 medicine. If you take birth control pills, switch to a non-oral method or add a barrier method for 4 weeks after starting and after each dose increase. Stop Zepbound® if you become pregnant.
The most common side effects are stomach-related. In the label's two main weight-loss trials, nausea affected 25% to 29% of people taking Zepbound®, compared with 8% on placebo. Learn more in our Zepbound® guide, or compare Wegovy® (semaglutide) with Zepbound® in our Wegovy® vs Zepbound® guide.
When to call a healthcare provider
Call your provider right away if you have severe stomach pain that won't go away, with or without vomiting or diarrhea that doesn't stop. The pain may spread to your back. Also call if you have signs of gallbladder problems, such as upper stomach pain, fever, or yellowing skin or eyes.
Tell your provider about signs of low blood sugar if you also take insulin or a sulfonylurea. If you take too much Zepbound®, call Poison Help at 1-800-222-1222.
Call 911 or go to the nearest emergency room if you have signs of a severe allergic reaction. These include swelling of your face, lips, tongue, or throat, or trouble breathing or swallowing.
Bottom line
- The Zepbound® KwikPen holds a month of weekly doses in one pen. It contains the same medicine as the single-dose pen, but it needs a new pen needle and priming before each dose.
- Throw it away after 4 doses or 30 days, whichever comes first, and never share it. Learn more on our obesity treatment hub.

Frequently asked questions
Is the Zepbound® "quick pen" the same as the KwikPen?
Yes, "quick pen" is how many people search for the Zepbound® KwikPen. KwikPen is Lilly's name for its multi-dose pens. The Zepbound® KwikPen holds 4 weekly doses of tirzepatide at one strength, from 2.5 mg to 15 mg.
Can you use the leftover medicine in a Zepbound® KwikPen?
No, you shouldn't use leftover medicine in a Zepbound® KwikPen. After 4 doses, the pen stops you from dialing a full dose. The label says to throw it away, even with medicine left, and never to move leftover medicine into a syringe.
What if you miss a Zepbound® KwikPen dose?
If you miss a dose, take it within 4 days (96 hours). If more than 4 days have passed, skip it and take your next dose on your usual day. Keep doses at least 3 days apart if you change your injection day.
Can you switch from the single-dose pen to the KwikPen?
Yes, you can switch to the KwikPen with a prescription written for it. The doses and medicine are the same. The FDA label says you should get training on the new device. This matters because the KwikPen needs a pen needle and priming.
Does the Zepbound® KwikPen cost more than other forms?
Lilly sells the KwikPen at the same self-pay price as Zepbound® single-dose vials. A KwikPen-only savings card gives that price at major pharmacies. With insurance, your cost depends on your plan's coverage for weight-loss medicines.
