How It Works: Two Signals
Here are those two hands up close. Semaglutide reaches for one hunger signal. Tirzepatide works two at the same time.

Now the big job: the scale. Same medicine, new approval. In November 2023 the FDA cleared Zepbound for chronic weight management (long-term weight loss) in adults with obesity, or who are overweight with a related health problem.1

Are Mounjaro® and Zepbound® the same drug?
Yes. Both are tirzepatide. The brand name tells you which use the FDA approved — Mounjaro® for type 2 diabetes, Zepbound® for chronic weight management and, now, sleep apnea.
Which brand is approved for weight loss?
Zepbound®. Mounjaro® is approved for type 2 diabetes, though a prescriber may use tirzepatide off-label (for a use the FDA has not officially approved). The active ingredient is identical.
How is tirzepatide different from semaglutide?
Tirzepatide copies two gut hormones (GLP-1 and GIP); semaglutide copies one. In the head-to-head SURMOUNT-5 trial, tirzepatide averaged about 20.2% body weight lost versus 13.7% for semaglutide over 72 weeks.
Is compounded tirzepatide the same thing?
No. Compounded tirzepatide (a version mixed by a pharmacy rather than made by the original drug maker) is not FDA-approved and was not part of the clinical trials referenced here. Only the brand-name versions carry FDA approval.
Are there serious risks to know about?
Both brands carry a boxed warning (the FDA’s most serious type of label warning) for thyroid C-cell tumors (a kind of thyroid growth seen in animal studies) and share common stomach side effects. Always talk to a licensed clinician before starting.
