What GLP-1 medicines are, in plain terms?
GLP-1 is a hormone your gut makes after you eat. It helps manage blood sugar and tells your brain you are full. Your body clears it within minutes.
GLP-1 medicines copy that natural hormone. They are built to last much longer, so one dose can work for days. That steady signal is the whole idea.
The effect is simple to picture. Food leaves your stomach more slowly, so you feel full sooner. Appetite signals quiet down, including what many people call food noise.
Some newer medicines copy a second gut hormone too. Zepbound® (brand of tirzepatide) acts on both the GIP and GLP-1 receptors. That dual action is why it is called a dual GIP/GLP-1 medicine.
The brands approved for weight loss
Two brands lead GLP-1 for weight loss in the US. Wegovy® (brand of semaglutide) and Zepbound® (brand of tirzepatide) are both FDA-approved for chronic weight management in adults.
The timing is recent. The FDA approved Wegovy® in 2021 and Zepbound® in November 2023. Both are once-weekly shots.
Saxenda® (brand of liraglutide) is an older option. It is a daily shot approved for weight management before the other two arrived.
Some famous names are not weight-loss brands. Ozempic® and Rybelsus® also contain semaglutide, and Mounjaro® contains tirzepatide. The FDA approves those three for type 2 diabetes, not weight loss.
Two brands anchor GLP-1 for weight loss in the US: Wegovy® and Zepbound®. Everything else builds from there.
The weight-loss trials, by brand
Each brand earned approval through a large, named clinical trial. The numbers below are averages, and results vary from person to person.
STEP 1 tested Wegovy® in 1,961 adults. Those on Wegovy® lost about 14.9% of their body weight over 68 weeks, versus 2.4% on placebo (Wilding 2021).
SURMOUNT-1 tested Zepbound® in 2,539 adults. Those on the 15 mg dose lost about 20.9% over 72 weeks (Jastreboff 2022). Average loss ranged from 16.0% to 22.5% across the three doses.
SCALE tested Saxenda® in 3,731 adults. Those on Saxenda® lost about 8.0% of body weight over 56 weeks, versus 2.6% on placebo (Pi-Sunyer 2015).
These trial numbers apply only to the FDA-approved branded products. They do not transfer to compounded semaglutide (not FDA-approved; not a generic of Wegovy® or Ozempic®). Our semaglutide monograph covers that topic in depth.

Who these medicines are for?
Wegovy® and Zepbound® are prescribed for adults with obesity. They may also fit adults who are overweight and have a weight-related condition, like high blood pressure.
They are not right for everyone. A licensed clinician reviews your health history first. Pregnancy and certain thyroid conditions are reasons to avoid them.

How people start?
Most people begin one of two ways. One path is your regular doctor or an obesity medicine specialist. The other is a telehealth program built around these medicines.
The basic steps look similar. You share your health history, a licensed clinician reviews it, and a prescription follows if it fits.
Doses usually start low and rise slowly over weeks. That slow climb, called titration, helps your body adjust. Regular check-ins are part of the plan.
What to expect with side effects and safety?
Most side effects involve the gut. The common ones are nausea, diarrhea, vomiting, and constipation. They often ease as your body adjusts and doses rise slowly.
Less common but more serious problems can happen. That is one reason a clinician follows you over time. Report severe or lasting stomach pain right away.
All three labels carry an FDA boxed warning. It describes a rare thyroid tumor seen in rodent studies (medullary thyroid carcinoma). People with a personal or family history of that cancer, or of MEN 2, should not use these medicines.
Weight loss on these medicines is a slow build, measured in months and guided by a licensed clinician.
